New Client & Patient Form

New clients are welcome to fill out the new patient form below. DUE TO OVERWHELMING DEMAND FOR SERVICES, WE HAVE HAD TO SLOW OUR RATE OF INITIAL VISITS. Please be patient & considerate with our front desk staff while scheduling. We may be unable to schedule your pet for an appointment for a few months.

We will do our best to continue to accept emergency patients as we are able. If you believe your pet is in severe pain, your pet has become immobile, or has any other emergent conditions, please be sure to notate that in the form.

Please fill out and submit the below form to let us know that you are interested in scheduling an initial appointment with Dr. Shaw for your pet. Our Client Care Coordinators will reach out to you to get you on our schedule.

Please be aware that Dr. Shaw may not be able to see any new patients at this time. We do our best to use a triage system to ensure pets in an emergency situation can see Dr. Shaw ASAP. If you believe your pet is in pain, your pet has become immobile, or any other severe conditions, please be sure to notate that in the form.
